Filling Out the Paper Forms

  • Application Form
      Applicants in the U.S. complete and return the form "Application for Admission to Graduate School" and Applicants applying from outside the U.S. complete and return the form "Admission Application for International Students" to the admissions office.

  • Application for Admission, Fellowship and/or Assistantship
      Please complete the "Application for Admission, Fellowship and/or Assistantship", even if you do not wish to apply for a Fellowship or Assistantship. If you have an email address, please include it with this form. This form also asks for a statement of purpose. In this essay you may wish to highlight accomplishments and other pertinent information not solicited in the rest of the application. Applicants who already have a masters degree or some graduate mathematics training should also indicate your mathematical interests to help us match applicants with faculty members having similar interests. Please send the form and the statement of purpose to the mathematics department.

  • GRE scores
      Arrange for ETS to send a report of your GRE general test scores directly to the office of admission. If you did not include these scores on the Application for Admission, Fellowship and/or Assistantship form, please send the department your scores as soon as they become available. Send either a xerox copy or email with the scores to msomers@ufl.edu.

  • Transcripts
      Arrange to have official copies of transcripts from all of your previous undergraduate and graduate work sent directly to the office of admissions. Also, please arrange to have copies (they need not be official) sent directly to the mathematics department.
  • Letters of Recommendation
      Choose three people who know you well to write letters of reference for you, and give each person one of the "Letter of Recommendation" forms, asking them to return the letter directly to us as quickly as possible. Please try to include at least two instructors from upper division or graduate level mathematics courses who will be able to comment on your mathematical abilities. They may write letters, if they prefer, instead of using the forms.

  • TOEFL score (International only)
      The University requires most international students to report a Test of English as a Foreign Language (TOEFL) score. The University and Board of Regents have mandated minimum scores. Arrange for ETS to send an official report directly to the office of admissions. If you did not include these scores on the Application for Admission, Fellowship and/or Assistantship form, please send the department your scores as soon as they become available. Send either a xerox copy or email with the scores to msomers@ufl.edu.

  • TSE score (International only)
      The Test of Spoken English (TSE) is not required of international students for admission; however, the mathematics department can only support one or two students each year who do not have a score of 45 or better on the TSE. Consequently, chances of receiving financial assistance in the form of a Graduate Teaching Assistantship are much better with a score of 45 or better on the TSE. Arrange for ETS to send a score report to the mathematics department.

  • Certification of Financial Responsibility (International only)
      Foreign students should also complete the "Certification of Financial Responsibility" and return it to the office of admissions. This form is necessary to issue I-20 and IAP-66 forms needed to apply for visas.

  • Application Fee
      Remit the application fee to the admissions office per the instruction on the Application form.
